The MedElite Group is seeking a friendly Primary Care NP/PA to join Infinite Medical P.C., a MedElite-backed network, providing comprehensive care to residents in long-term care facilities in Cleburne, TX.

As a Primary Care Provider, you will work part-time (three days per week) and collaborate with an interdisciplinary team to manage chronic conditions, coordinate care, and promote person-centered care within a nursing home setting.

Qualifications

• Nurse Practitioner or Physician Assistant license.
• DEA license.
• 6 months experience in long-term care.

Responsibilities

• Provide comprehensive healthcare services and coordinate patient care.
• Work closely with a team of healthcare professionals.
• Assess, diagnose, and treat residents' physical and mental well-being.
• Conduct thorough patient assessments, including medical history reviews and physical examinations.
• Order and interpret diagnostic tests and develop treatment plans.
• Prescribe medications and administer treatments as part of care plans.
• Counsel residents and their families on health and wellness.

Responsibilities:

• Provide comprehensive healthcare services and coordinate patient care
• Work closely with a team of healthcare professionals
• Assess, diagnose, and treat residents' physical and mental well-being
• Conduct thorough patient assessments, including medical history reviews and physical examinations
• Order and interpret diagnostic tests
• Develop accurate diagnoses and treatment plans
• Prescribe medications and administer treatments
• Provide counseling to residents and their families
• Collaborate with the interdisciplinary team for seamless coordination of care
• Manage chronic conditions and monitor progress
• Make appropriate adjustments to care plans
• Commitment to person-centered care
• Create a safe and nurturing environment for elderly residents in the nursing home.

Requirements:

• Nurse Practitioner or Physician Assistant License
• DEA License
• 6 months experience in long-term care
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
