Job summary

Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care in long-term care facilities in Granbury & Mineral Wells, TX. This full-time role offers a competitive annual salary and a collaborative team environment.

The provider will perform exams, manage chronic and acute conditions, prescribe medications, and work with interdisciplinary staff to deliver patient-centered care that improves residents' quality of life.

Qualifications

• Active and unrestricted NP/PA license in TX.
• State license in good standing in Texas.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Strong communication, clinical and critical thinking skills.
• Must be able to lift 20 pounds and stand/walk up to 6–8 hours per day.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful and supportive environment for residents.

• Passion for elder care and person-centered healthcare
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
• Competitive salary per annum: $150,000 - $200,000
