Base pay range

$130,600.00/yr - $183,000.00/yr

About This Role

If you are passionate about serving seniors and helping them maintain independence, join our team! You will provide direct medical care to PACE participants as part of a collaborative Interdisciplinary Team, developing care plans that truly impact lives. You will meet participants where they are—home, clinic, virtual—and support their health and wellness.

Success in this role involves:
  • Conducting assessments and contributing to care plans
  • Building relationships and managing a panel of participants
  • Evaluating and adjusting medications and treatments
  • Documenting interactions in our EMR system
  • Referring to specialists and coordinating care
  • Educating participants on health management and advanced care planning
  • Providing care at various locations and supporting continuity of care
  • Collaborating with team members for holistic care
Ideal Candidate Profile:
  • Passionate about serving high-risk seniors
  • Experience with complex and older populations (minimum two years as RN/PA)
  • Excellent communication skills
  • Team-oriented with leadership qualities
  • Master’s degree and certification as NP or PA
  • Active, unrestricted license and CPR certification
  • Licensing in California if applicable; valid driver’s license and transportation
  • Bilingual in English and Spanish preferred
  • Knowledge of geriatric and chronic care issues
Additional Information:
  • Location: Los Angeles (90057) or East Compton (90221)
  • Hours: 8 am - 5 pm with rotating on-call
  • Travel within service area as needed
  • Sign-on bonus: $15,000
