Qualifications

• Current board certification for NP or PA.
• California medical license and DEA registration.
• BLS Certification required; 30-day grace period to obtain if not current.

Responsibilities

• Provide triage, care management, and patient education for panel of patients.
• Refer to specialty providers and ensure follow-up on bio-psychosocial needs.
• Prescribe medications, order tests, and manage preventive and chronic care.
