Job summary

La Clínica de La Raza is seeking a qualified NP/PA to deliver patient-centered primary care in an outpatient setting within the Bay Area clinics. You will work in a collaborative team to provide preventive and chronic care, with emphasis on culturally competent service to diverse communities.

Must hold a California license and DEA; BLS is required. Flexible schedules and opportunity to impact health equity are offered, with a focus on mentoring and professional growth.

Qualifications

• Current BLS certificate is required.
• Board certification or plan to become certified within 6 months of hire.
• Valid California license and DEA are required.

Responsibilities

• Provide medical triage and care management for a patient panel.
• Make referrals to specialists and ensure follow-up for bio-psychosocial needs.
• Prescribe medications and order diagnostic tests; collaborate with cross-functional team.
