Job summary

Vera Whole Health in Las Vegas, NV is seeking a Primary Care Advanced Practice Provider (APRN) to deliver comprehensive, patient-centered care across the continuum. You will collaborate with physicians and an interdisciplinary care team to manage acute, chronic, and preventive care needs, develop individualized care plans, diagnose and treat conditions, and coordinate services to improve outcomes.

Qualifications

• Master’s or Doctor of Nursing Practice from an accredited program.
• Current, unrestricted NP license in the applicable state.
• Board certification appropriate to practice setting (FNP/AGPCNP/AGACNP).
• DEA registration and BLS certification.
• 2+ years advanced practice experience in primary care preferred.
• Knowledge of primary care medicine and value-based care.

Responsibilities

• Provide evidence-based medical care meeting diverse patient needs.
• Assess populations and develop plans of care with the care team.
• Ensure comprehensive care across pediatrics to geriatrics and women's health.
• Collaborate on quality improvement and population health initiatives.
• Care for 12-16 patients per day across preventive, chronic, and acute care.
• Participate in after-hours on-call rotation and team huddles.
• Document and code timely to reflect care, risk, and HCCs.
