Job summary

Vera Whole Health in Las Vegas, NV seeks a Primary Care Advanced Practice Provider (APRN) to deliver comprehensive, patient-centered care across the continuum with a collaborative care team. You will manage acute, chronic, and preventive care, perform assessments, diagnose and treat, and coordinate services to optimize outcomes.

Qualifications

• Master's or Doctor of Nursing Practice degree from an accredited Nurse Practitioner program required.
• Current, unrestricted Nurse Practitioner license and Advanced Practice Registered Nurse licensure in the applicable state required.
• National board certification as a Family Nurse Practitioner (FNP), AGPCNP, AGACNP, or other certification.
• Current DEA registration and BLS certification required.
• Typically requires two or more years of advanced practice clinical experience in primary care or related.
• Primary care experience is strongly preferred.
• Knowledge of primary care medicine, preventive guidelines, chronic disease management, and value-based ambulatory care.
• Experience using EHRs and population health platforms to support quality care and regulatory compliance.

Responsibilities

• Provide primary care medical services to 12–16 patients per day.
• Collaborate with the care team to develop and implement care plans.
• Participate in quality improvement and process development.
• Coordinate care with home health, specialists, and community resources.
• Document and code timely to reflect care provided and HCCs addressed.
