Harbor Health in San Antonio, TX is seeking a Nurse Practitioner or Physician Assistant to lead care in a multi-specialty clinic at the Babcock practice. You will deliver high-quality primary care, collaborate with a clinical team, and contribute to a payvider model focused on effective patient outcomes.

The role demands licensure as a PA or NP in Texas, NCCPA/ANCC/AANP certification, DEA and CPR credentials, and a patient-centered approach with strong teamwork skills.

Qualifications

• Graduate of an accredited Physician Assistant Program and certified by NCCPA or Nurse Practitioner Program and certified by ANCC or AANP.
• Current Texas licensure to practice as PA/NP.
• Licensed and certified in Family Medicine.
• Current DEA and CPR certification.
• Valid Texas Driver's License.
• Ability to meet credentialing and privileging requirements.
• Comfortable working as a solo provider within a clinic team.

Responsibilities

• Interview members to obtain medical history, perform exams, order tests, and prescribe medications.
• Provide continuity in managed care for members with chronic conditions.
• Make referrals for secondary and tertiary care in collaboration with the care team.
• Perform procedures according to privileges and provide health maintenance visits.
