Job summary

Thundermist Health Center is seeking a Nurse Practitioner or Physician Assistant for primary care across Rhode Island. Manage your own patient panel, deliver continuity-focused care, and work within an integrated care model with physicians, behavioral health clinicians, care managers, and community health workers.

You'll provide preventive, acute, and chronic disease management in a multi-site FQHC, supporting patients regardless of insurance or income.

Qualifications

• Master’s degree from an accredited NP program with RI APRN licensure or Master’s from an ARC-PA PA program with RI PA licensure.
• National certification in Family Medicine, Internal Medicine, Women’s Health, or Pediatrics.
• Current DEA registration and RI Controlled Substance Registration.
• Current BLS; ACLS, PALS, NR P as applicable to specialty.
• Eligibility to enroll with Medicare, Medicaid, and commercial payers (CAQH).
• Working knowledge of electronic health records; eClinicalWorks preferred.

Responsibilities

• Provide primary medical care to an assigned patient panel including visits, acute care, and chronic disease management.
• Perform comprehensive histories, exams, and diagnostic assessments; order/interpret labs and imaging.
• Prescribe medications within scope of practice; deliver anticipatory guidance and patient education.
• Coordinate referred services; triage concerns and determine level of care/transfer.
• Collaborate with physicians, APPs, nurses, care managers, and community health workers.
• Supervise NP/PA and medical students as assigned.
