Job summary

Thundermist Health Center in Woonsocket is seeking a Primary Care and Cross-Coverage Hybrid Nurse Practitioner to lead a dedicated patient panel while also covering colleagues’ administrative tasks when they are out of the office.

You will provide autonomous, guideline-driven primary care and support cross-coverage to ensure continuity of care for underserved populations. This role is based in Thundermist’s Woonsocket clinic with some evenings.

Qualifications

• Board Certified Family Nurse Practitioner (FNP)
• RI license to practice as NP in good standing
• 3+ years of post-graduate primary care experience
• Excellent oral and written communication with medical terminology
• Commitment to health equity and community health

Responsibilities

• Provide complete management of acute and subacute medical care for scheduled patients
• Deliver face-to-face primary care by appointment for an assigned panel
• Review and manage medical records
• Coordinate referred services (lab, specialty care, pharmacy, etc.)
• Exercise independent professional clinical judgement within guidelines
• Cross-cover clinical administrative tasks for colleagues out of the office
• Review documents, results, messages, and phone encounters in the EHR
• Participate in departmental and team meetings
• Help review and develop policies and clinical goals

About the Role

Are you a Nurse Practitioner who wants the continuity of your own primary care panel along with the variety of clinical cross-coverage? Thundermist Health Center is looking for a Primary Care and Cross-Coverage Hybrid Nurse Practitioner (“Hybrid NP”) to join our primary care practice. Part of your time will be spent providing face-to-face primary care for a panel of patients as a licensed, autonomous NP exercising professional clinical judgement within the health center’s guidelines, policies, and procedures. The remainder of your time is reserved for cross-covering the clinical administrative work of colleagues who are out of the office — reviewing and acting on documents, results, messages, and phone encounters in the electronic health record. What You’ll Do

Primary Care

• Provide complete management of acute and subacute medical care for scheduled patients

• Participate in departmental, team, and full staff meetings
• Help review and develop policies and procedures, health care plans, and clinical goals and objectives

What We’re Looking For

You bring clinical expertise, sound judgement, and genuine compassion to every patient encounter. You’re an autonomous provider who values long-term patient relationships and is equally comfortable managing a full primary care schedule and keeping your colleagues’ clinical inboxes moving.

• Board Certified Family Nurse Practitioner
• Full, unrestricted Rhode Island licence to practice as a Nurse Practitioner
• Three or more years of post-graduate medical practice in primary care; experience in a Federally Qualified Health Center preferred
• Exceptional oral and written communication skills with proficiency in medical terminology
• A genuine commitment to patient-centred, high-quality care for diverse and underserved populations, with an authentic passion for health equity and community health

Schedule & Location

This is a full-time (40+ hours/week) position based in Thundermist’s Woonsocket primary care practice; part-time status may be considered if agreed upon by administration. The work schedule will involve some evenings. Some after-hours phone call coverage is included.
