Job summary

One Medical is seeking a qualified Advanced Practitioner for a full-time, in-person role in Fairfield County, CT. You will manage a diverse patient panel, provide acute and chronic care, and conduct tele-health visits as needed.

The role includes ongoing professional development through weekly Clinical Rounds and collaboration with both in-person and virtual teams. Required: accredited NP/PA program with national certification and CT license by start date; 2+ years outpatient primary care or 1

Qualifications

• Completed an accredited NP or PA program with a national certification.
• State licensed in CT by start date.
• In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in an outpatient primary care setting, OR 1 year in outpatient primary care plus either a 1 year primary care fellowship or 1+ year in an urgent care setting.

Responsibilities

• Manage a patient panel with a broad array of patient needs; conduct acute, chronic, and well visits.
• Treat patients in-office or in testing centers and conduct occasional tele-health visits.
• Participate in weekly Clinical Rounds and other learning modalities.
• Collaborate with in-office teammates and virtual clinical teams daily.
• Utilize clinical training and perform in-office procedures as appropriate.
• Be willing to obtain additional state licensure and credentialing for virtual primary care in additional states.

Education

Accredited NP or PA program with national certification

CT state license

5 years outpatient primary care experience (2 years if no fellowship) OR 1 year with fellowship/1+ year urgent care
