Job summary

Qualifications

• Master's degree in nursing with NP certification.
• National Board Certification for Nurse Practitioners.
• Practicing-state licensure as a Nurse Practitioner.

Responsibilities

• Record patient history and perform physical assessments.
• Develop therapeutic care plans and order diagnostic tests.
• Interpret test results and document medical records.
• Coordinate patient care with family, community services and team.
• Prepare prescriptions and administer medications; wound care.
• Educate patients on health, medications and preventive care.
• Triage calls and monitor ongoing patient care.
• Participate in quality improvement initiatives.
• Assist with outpatient concerns as needed.
