Schodack Internal Medicine and Pediatrics in Castleton-on-Hudson, NY is seeking a Primary Care Nurse Practitioner or Physician Assistant to join a collaborative, physician-led multispecialty network. The role focuses on comprehensive care across the lifespan, including wellness visits and chronic disease management.

Qualifications

• License for Physician Assistant in NYS and active registration.
• NP certification by AANPCB or ANCC, and NYSED-acceptable program.
• CPR certification is required.
• Minimum ambulatory care experience for PA or NP as specified in requirements.

Responsabilités

• Provide comprehensive primary care to patients across the lifespan.
• Perform wellness visits, physicals, acute visits, and follow-up care.
• Evaluate, diagnose, and manage acute and chronic conditions.
• Provide preventive health counseling and patient education.
• Coordinate care with physicians, specialists, and other team members.
• Maintain accurate and timely electronic medical records.

Formation

Physician Assistant licensure + NYS registration

Nurse Practitioner certification (AANPCB/ANCC)

CPR certification

Ambulatory health care experience (PA or NP)
