City Different in Santa Fe, NM is seeking an Advanced Practice Provider (NP/PA) to join a growing, patient-focused primary care team.

You will build your own patient panel, work with a collaborative physician and clinical staff, and provide comprehensive preventive care while staying engaged through mentoring and ongoing learning. Schedule is Monday–Friday, 7:30 a.m.–4:00 p.m., with no nights and minimal call.

Qualifications

• Active New Mexico license as NP or PA.
• DEA license or ability to obtain within 30 days.
• Experience in primary care or eager new graduate.

Responsabilités

• Provide comprehensive, compassionate primary care to a diverse patient population.
• Evaluate patients, diagnose conditions, order and interpret diagnostic testing, and develop treatment plans.
• Focus on preventive care, wellness, and patient education.
• Build your own patient panel and develop long-term relationships with patients.
• Collaborate with physician and clinical team to provide exceptional care.
• Document patient encounters accurately in the EMR.
• Educate and empower patients to take an active role in their health.
• Contribute ideas as the practice grows and evolves.
