Job summary

Rochester Regional Health is seeking an experienced Nurse Practitioner in Internal Medicine for outpatient primary care of adult patients. You will work with a collaborative care team to manage acute and chronic conditions, promote preventive care, and build lasting patient relationships.

The position features a Monday–Friday daytime schedule in Linden Oaks with comprehensive benefits and a supportive work environment.

Qualifications

• MS in Nursing is required.
• Nurse Practitioner Certification with NYS licensure.
• RN license with NYS Office of Professions.

Responsibilities

• Provide comprehensive primary care to adult patients, including evaluation, diagnosis and management of acute and chronic conditions.
• Develop treatment plans, prescribe medications, order and interpret diagnostic tests, and provide preventive care and patient education.
• Collaborate with physicians, APPs and other members of the healthcare team to coordinate care and specialty referrals as needed.
• Maintain timely and accurate documentation of patient visits, diagnoses and treatment plans.
