Job summary

Rochester General Hospital in Linden Oaks seeks a Nurse Practitioner in Internal Medicine to provide comprehensive primary care to adult patients in an outpatient setting. You will work with a collaborative care team to manage acute and chronic conditions, promote preventive care, and build lasting relationships with patients.

This full-time role offers a Monday–Friday schedule, emphasis on documentation, care coordination, and patient education while meeting NYS NP standards and maintaining

Qualifications

• MSN or equivalent nursing degree required.
• NYS Nurse Practitioner certification required.
• Active RN licensure in New York State.

Responsibilities

• Provide comprehensive primary care to adult patients, including evaluation, diagnosis and management of acute and chronic conditions.
• Develop treatment plans, prescribe medications, order/interpret tests, and provide preventive care and education.
• Collaborate with physicians, APPs and team members to coordinate care and referrals.
• Maintain timely and accurate documentation of patient visits, diagnoses and plans.
