Job summary

Vera Whole Health in South Tacoma, Washington is seeking a licensed NP or PA to practice primary care with time for meaningful patient relationships. You will see 12-16 patients per day within a collaborative care team, receiving longer appointment times and support from a Whole Health Coach to help patients achieve their health goals.

Qualifications

• Current, unrestricted NP or PA license (or ability to obtain prior to start).
• Board Certification as an NP or PA.
• Current DEA registration.
• Primary care experience preferred.

Responsibilities

• Deliver evidence-based primary care services in a patient-centered environment.
• Build trusted relationships and help patients achieve their health goals.
• Partner with patients and the care team, including a Whole Health Coach, to develop personalized care plans.
• Provide preventive care, acute care, chronic disease management, and care coordination.
• Participate in team meetings and quality improvement initiatives that drive exceptional patient outcomes.
