Job summary

Lake Superior Community Health Center is seeking a qualified Nurse Practitioner or Physician Assistant to provide primary care across the lifespan, emphasizing preventive care and management of acute and chronic conditions.

Responsibilities include patient assessment, diagnostics, care coordination, documentation in the EHR, and adherence to HIPAA/OSHA. Minimum 2 years in community health or primary care; CPR required. Eligibility for licensure in MN and WI.

Qualifications

• Graduate of accredited Nurse Practitioner or Physician Assistant program.
• Minimum of 2 years’ experience in community health or primary care setting.
• CPR certificate.

Responsibilities

• Perform history taking, physical examination, and treatment planning within scope of licensure; provide preventive care.
• Order, interpret and evaluate diagnostic tests; refer to staff physicians and specialists as needed.
• Coordinate and manage patient care in consultation with staff physicians and other health care team members.
• Maintain documentation in the EHR according to clinical guidelines and procedures.
• Comply with organizational policies, HIPAA and OSHA regulations.
• Actively participate in staff meetings and quality improvement activities.
