• Develop individualized treatment plans including medication management and preventive services
• Perform and interpret routine diagnostics and office-based procedures within scope
• Coordinate referrals and collaborate with multidisciplinary teams
• Educate patients and caregivers to promote self-management and healthy outcomes
• Document encounters and outcomes in the electronic health record
