Qualifications

• Evaluate, diagnose, and manage patients across acute illnesses, chronic diseases, and preventive care needs.
• Develop individualized treatment plans including medication management and lifestyle counseling.
• Collaborate with physicians and interdisciplinary teams to improve outcomes and care coordination.

Responsibilities

• Evaluate, diagnose, and manage patients presenting with acute illnesses, chronic diseases, and preventive healthcare needs.
• Develop individualized treatment plans that include medication management, preventive services, lifestyle counseling, health promotion, immunizations, and ongoing monitoring of outcomes.
• Coordinate referrals to specialty care, behavioral health, and community resources as clinically indicated.
• Document encounters and outcomes in the electronic health record following organizational standards.
