Job summary

The Veterans Health Administration, Department of Veterans Affairs, seeks an Advanced Practice Nurse Practitioner (CNP) for the Primary Care Service at the VA Providence Healthcare System. This is a full-time, 40-hour position with the Education Debt Reduction Program (EDRP) eligibility and program support.

Responsibilities include managing a patient panel with acute and chronic conditions, leading care delivery, and coordinating with interdisciplinary teams.

Qualifications

• Licensure: 1 full and unrestricted license from any US State or territory.

Responsibilities

• Manages a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
• Orders and interprets diagnostic studies; Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals/consults to specialty services; Provides education to patients and families; Adjusts plan of care as needed to achieve optimal outcomes.
