Job summary

The Veterans Health Administration, Department of Veterans Affairs, seeks an Advanced Practice Nurse Practitioner (CNP) for the Primary Care Service at the VA Providence Healthcare System. This is a full-time, 40-hour position with the Education Debt Reduction Program (EDRP) eligibility and program support.

Responsibilities include managing a patient panel with acute and chronic conditions, leading care delivery, and coordinating with interdisciplinary teams.

• Initiates referrals/consults to specialty services; Provides education to patients and families; Adjusts plan of care as needed to achieve optimal outcomes.

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The Advanced Practice Nurse Practitioner (CNP) position is in the Primary Care Service at the VA Providence Healthcare System
• This position is full-time at 40 hours
• Major duties include - but are not limited to: Manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Orders and interprets diagnostic studies Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals/consults to specialty services Provides education to patients and their families Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Leads the delivery of care to assure continuity of care and peer accountability for practice - improving outcomes of panel/program
• Engages in evidence-based practice and the application of clinical practice guidelines
• Implements quality improvement initiatives that lead to outcomes at the panel/program level Other duties assigned
