Woodlands Medical Specialists in Pensacola, FL seeks an experienced Nurse Practitioner to join our primary care team. You will deliver outpatient care to adult patients, manage chronic conditions, provide preventive services, and collaborate with physicians and staff to ensure high-quality, patient-centered care.

The ideal candidate holds a Florida APRN license, NP certification, prescriptive authority, and DEA registration.

Qualifications

• Current and unrestricted Florida APRN license.
• National certification as NP.
• Prescriptive authority to practice in Florida.
• Current DEA registration.
• Current BLS certification.
• Graduate of an accredited Nurse Practitioner program.
• Primary care experience preferred.
• EMR experience preferred.
• Experience with Medicare/Medicare Advantage/Medicaid and commercial insurance populations preferred.
• Experience with chronic disease management and preventive care preferred.

Responsabilités

• Conduct comprehensive patient histories and physical examinations.
• Diagnose and treat acute and chronic conditions.
• Develop and implement individualized treatment plans.
• Order, interpret, and follow up on laboratory and diagnostic studies.
• Prescribe medications and treatments in accordance with Florida law and practice protocols.
• Provide preventive healthcare services, including health screenings and immunizations.
• Manage chronic conditions such as diabetes, hypertension, and COPD.
• Perform office-based procedures within training scope.
• Provide patient counseling on medications and lifestyle modifications.
• Maintain accurate EMR documentation.
• Coordinate referrals and follow-up with specialists and other providers.
• Support CCM and RPM programs and value-based care initiatives.
• Collaborate with revenue-cycle teams for documentation and coding.
