Samaritan Daytop Health is a FQHC community‑based health center based in the Bronx that provides holistic, integrated primary and behavioral healthcare to adults and families. It is in collaboration with Samaritan Daytop Village which provides housing, addiction support and behavioral health to the community. The clinic offers comprehensive services including primary care, psychiatric care, preventive health services, nutritional counseling, and case management, with a strong focus on culturally responsive and affordable care for underserved populations. As a trusted partner of Samaritan Daytop Village, the health center is closely integrated with substance use, mental health, and recovery programs, allowing patients to receive coordinated physical and behavioral healthcare in a supportive, patient‑centered environment.

We are seeking a Primary Care Physician (MD) to join our new Primary Care Health Center. This location offers a collaborative, integrated care environment with complementary programs and services designed to support our clients comprehensively. The physician will be part of a multidisciplinary team committed to delivering high‑quality, patient‑centered care.

Responsibilities

• Mentoring and supporting the care team to enhance professional growth and team efficiency.
• Perform history and physical examinations in accordance with current standards of professional practice, regulatory requirements and organizational policies.
• Deliver patient‑centered primary care while integrating preventive, chronic, and acute care strategies.
• Reviewing and following up on labs, imaging, referrals, and hospital/ED transitions.
• Documenting clinical encounters accurately in the electronic health record.
• Assess patients’ condition and age‑specific medical needs; set goals; prescribe appropriate clinical interventions to meet those goals that reflect the physical, psychological, social, cultural and rehabilitative needs of the patient.
• Provide complete and appropriate documentation regarding clinical observations, diagnoses, treatment objectives, and patient responses to treatment regimens. Will utilize an outcome‑oriented approach.
• Prescribe appropriate laboratory/diagnostic procedures related to the patient’s health status and facilitate appropriate treatment interventions predicated on results of these procedures.
• Perform direct patient care and procedures in accordance with SDH policies and procedures.

Qualifications

• A graduate of an accredited medical school.
• Currently registered to practice as a Physician in New York State with a specialty in Family Medicine or Internal Medicine.
• Recent experience in an out‑patient care setting and experience in managing care of the substance use population.
• Possess:
  • NYSED MD/DO license.
  • Board Certified.
  • Valid DEA registration.
  • BCLS/ACLS/PALS certifications.
  • eMEDNY Provider or willingness to enroll as a “Provider” upon hire.
  • Active NPI.

Benefits

This is a full‑time exempt role with a compensation of $210,000 - $265,000 annually with a full benefits package. The compensation offered for this role is dependent on experience and credentials.

This clinic is located inside the Richard Pruss building in the Port Morris section of the South Bronx and has a regular schedule of Monday‑Friday 9am‑5pm. Due to clinic operating hours, flexibility is required to work occasionally outside of 9am‑5pm.
