Qualifications

• LPN license with primary care experience.
• Ability to perform vitals, histories, injections, and basic procedures.
• Proficiency in EHR documentation with accuracy.
• Strong patient communication and education skills.
• Team-oriented and able to coordinate care with physicians.

Responsibilities

• Room patients and obtain vital signs.
• Collect/update medical histories and complete medication reconciliation.
• Administer injections and perform basic procedures.
• Support chronic disease management initiatives.
• Document care in the EHR.
• Assist with care coordination and provide patient education.
• Collaborate with physicians and interdisciplinary teams to support individualized care.
