The LPN/MA position provides clinical support and nursing services to clients and patients in a variety of healthcare settings, including primary care, behavioral health, substance use treatment programs, residential facilities, crisis services, outpatient clinics, and community-based environments. This role is responsible for conducting assessments, coordinating care, providing health education, managing medications, supporting providers during examinations, monitoring client health needs, and maintaining accurate clinical documentation while collaborating with interdisciplinary treatment teams.

Key Responsibilities
- Conduct nursing assessments, screenings, health histories, vital signs, and ongoing evaluations to support quality patient care
- Collaborate with interdisciplinary treatment teams to coordinate services and develop individualized care and action plans
- Educate clients, patients, families, and guardians regarding health conditions, medications, treatment plans, disease prevention, and wellness strategies
- Monitor patient health status, identify changes in condition, and coordinate appropriate interventions as needed
- Administer and monitor medications according to organizational policies, physician orders, and regulatory requirements
- Assist providers with examinations, treatments, injections, laboratory collections, and specialized clinical procedures
- Coordinate medical appointments, specialty services, and healthcare resources to support patient treatment needs
- Monitor medication effectiveness, side effects, laboratory results, and treatment modifications while communicating with providers and pharmacies
- Support infectious disease control efforts, health monitoring, and client hygiene initiatives
- Arrange and monitor dietary accommodations related to medical conditions when appropriate
- Maintain timely, accurate, and thorough clinical documentation in the electronic health record
- Respond professionally and effectively during behavioral health and medical crisis situations
- Promote organizational goals and contribute to a positive, patient-centered healthcare environment
- Perform additional duties as assigned by program leadership
Education, Experience, and/or Credential Qualifications
- Current Licensed Practical Nurse or Medical Assistant license in the state of practice
- Must be willing to become licensed in additional states beyond the state currently licensed in
- Must be willing and comfortable providing telehealth services to individuals served
- BCLS certification required

- Current driver's license, acceptable driving record, and current automobile insurance required
- Demonstrated ability to communicate effectively with clients, families, providers, staff, and community partners
- Strong time management, organizational, and multitasking skills
- Ability to maintain confidentiality and exercise sound professional judgment
- Ability to develop and maintain positive working relationships with diverse populations
- Comfortable working in fast-paced and evolving healthcare environments
- Ability to remain calm, professional, and effective during crisis situations
- Knowledge of nursing principles, behavioral health services, substance use treatment, and healthcare regulations
- Ability to effectively utilize computers, electronic health records, and related software applications
- Successful completion of required background screenings, including criminal history, abuse/neglect, fingerprint, and driving record checks
Physical Requirements
- Perform sedentary work involving occasional exertion of up to 10 pounds of force
- Sit for extended periods while completing documentation and administrative duties
- Occasionally stand and walk to support patient care activities
- Perform repetitive hand, finger, and arm movements for typing, charting, and clinical documentation
- Safely push, pull, lift, and move lightweight objects as needed to perform essential job functions
- Maintain the physical ability to fulfill clinical responsibilities in a healthcare environment with occasional movement throughout the workday
