Job summary

Evergreen Family Medicine in Oregon seeks a Licensed Behavioral Health Consultant to provide on-site, telehealth and community behavioral health services within the primary care team. You will conduct assessments, implement brief CBT-based interventions, and support care coordination with providers to address psychiatric and psychosocial concerns.

Responsibilities include rapid access for providers, crisis intervention as needed, and use of screening data to guide care.

Qualifications

• Master's degree required with relevant clinical training.
• Two years of clinical service delivery preferred, especially in primary care.
• Licensure in Oregon or eligibility to obtain independent licensure.

Responsibilities

• Provides on-site, telehealth and community behavioral health services in a primary care clinic.
• Maintains a schedule that ensures immediate access for primary care providers.
• Provides initial assessment and behavioral interventions for referred patients.
• Uses brief CBT and evidence-based practices to guide interventions.
• Conducts crisis intervention and supports referrals as appropriate.
