Job summary

Harbor Care is seeking a dedicated Primary Care Advanced Practice Registered Nurse (APRN) to provide patient-centered care at our Nashua, NH health center. This full-time, on-site role includes 32 hours of direct patient care weekly and 8 hours of admin time, with a $10,000 sign-on bonus and loan forgiveness eligibility.

Qualifications

• Master’s degree in Advanced Practice Nursing required.
• New Hampshire Nurse Practitioner license required.
• DEA registration and national NP certification required.

Responsibilities

• Diagnose and treat acute and chronic medical problems for patients of all ages.
• Diagnose and treat opioid use disorder and other addictions.
• Be part of an integrated care team serving diverse populations, including uninsured and homeless individuals.
• Provide comprehensive health assessment, diagnosis, treatment and referrals.
• Lead or participate in development of individualized treatment plans with the care team.
