Job summary

Harbor Care seeks a dedicated Primary Care Advanced Practice Registered Nurse (APRN) to provide comprehensive, patient-centered care at our community-based health care center in Nashua, NH. This full-time, salaried role focuses on diagnosing and treating primary care patients within an integrated care team that serves uninsured, homeless, and underserved populations.

Qualifications

• Must hold a Master's degree in Advance Practice Nursing.
• Experience delivering primary care to patients of all ages.
• Strong interpersonal, clinical, and communication skills.

Responsibilities

• Diagnose and treat acute and chronic medical problems across ages.
• Diagnose and treat opioid use disorder and other addictions.
• Be part of an integrated care team serving underserved populations.
• Provide comprehensive health assessment, diagnosis, treatment, and referrals.
• Lead and participate in multidisciplinary teams to develop treatment plans.
