Qualifications

• Current, unrestricted Florida license as APRN or PA.
• Master's Degree in Nursing (APRN) or graduate PA program.
• Ability to practice within Florida scope-of-practice guidelines.
• Strong clinical judgment, communication, and patient education skills.

Responsibilities

• Provide comprehensive primary care services to an assigned patient panel in collaboration with physicians and the care team.
• Conduct patient assessments, develop individualized care plans, and implement appropriate treatment interventions.
• Diagnose and manage acute and chronic health conditions using evidence-based practices.
• Interpret diagnostic testing, including laboratory and imaging results.
• Prescribe medications and treatment plans within your scope of practice.
• Educate patients and families on disease management, preventive care, and healthy lifestyle choices.
• Coordinate care across providers, specialists, caregivers, home health agencies, and community resources.
• Support advanced care planning and palliative care discussions for patients with chronic or life-limiting illnesses.
• Help address quality measures, risk adjustment (HCC) documentation, and patient outcomes initiatives.
• Reduce avoidable emergency department utilization through proactive care management.
• Ensure accurate, timely documentation and billing in compliance with organizational and regulatory standards.
• Collaborate closely with physicians and consult on complex diagnoses or treatment decisions as needed.
