Job summary

Yale New Haven Health is seeking a Primary Care APP Float (NP or PA) to provide flexible outpatient coverage across Fairfield County offices. The role supports ambulatory clinics with triage, assessment, and management of patient needs, in collaboration with supervising physicians.

You will work Monday–Friday, 8:00–4:30 or 8:30–5:00, with a focus on continuity of care without hospital rounds or on-call duties. EMR: EPIC.

Qualifications

• Graduate from an accredited Nurse Practitioner or Physician Assistant program.
• Active clinical certification (NP/PA) and state licensure.
• Experience in outpatient care and interprofessional collaboration.

Responsibilities

• Provide assessment, triage and therapeutic management in outpatient clinics as part of a care team.
• Coordinate with supervising physicians across IM and FM practices.
• Document encounters accurately in the EMR and support quality care initiatives.
