Job summary

Yale New Haven Health is seeking a Primary Care Outpatient APP Float to join our network across Fairfield and New Haven counties. The APRN/PA will provide assessment, triage, and therapeutic management as part of an interdisciplinary team in ambulatory settings, spanning IM and FM practices.

No hospital duty, no patient panel, flexible schedules, and assignment could extend beyond three months. Expect collaboration, documentation, and ongoing professional development within our physician-led

Qualifications

• Graduate from an accredited NP or PA program.
• Current licensure and certification as NP or PA.
• Hospital and clinical setting experience preferable.

Responsibilities

• Provide assessment, triage, and therapeutic management of patient/family needs as part of an interdisciplinary team.
• Educate patients and families on care and available resources.
• Ensure accurate documentation per hospital and regulatory requirements.
• Collaborate with supervising physicians and follow state regulations and hospital policies.
• Support professional development and participate in quality and process improvement initiatives.
