AdventHealth Primary Care+ in Florida is seeking Advanced Practice Providers to join our team-based primary care model, delivering whole-family care and continuity with patients in a collaborative environment that supports high-quality care and work–life balance.

In this role, you will work with other Advanced Practice Providers and care teams to provide preventive care, chronic disease management, and long-term patient relationships in a modern, well-supported clinic.

Qualifications

• Active Florida APRN or PA-C license
• Experience in primary care or family medicine preferred
• Strong preventive care and chronic condition management skills
• BLS certification required

• Provide comprehensive primary care in a team-based model
• Collaborate with APs and care teams to deliver preventive care and chronic disease management
