Qualifications

Responsibilities

• Coordinate primary care health services and maintain clinic schedule of patients.
• Perform clinical assessments, history taking, and evaluations.
• Participate in daily team transition huddles for patient care.
• Prescribe appropriate pharmacologic and non-pharmacologic treatments.
• Ensure quality of care through consultation, collaboration, and evaluation.
