Job summary

Kids for the Future in Kingston, NY is seeking a Community Health Worker to identify and assist families needing preventive and primary health care, with a focus on entry into prenatal care for pregnant women and improved birth outcomes.

The CHW will provide basic case management services including health assessment, education, referral to services, and ongoing support, serving as liaison between families and community agencies to ensure access to essential care.

Qualifications

• Familiar with available community resources and knows how to access services appropriate to participants’ needs.
• Proficient in MS Word, Excel and PowerPoint.
• Demonstrated organizational, interpersonal, oral and written communication skills and ability to handle multiple assignments.
• Ability to generate, interpret and analyze data from multiple sources.
• Bilingual Spanish/English preferred.
• Demonstrated experience in case management preferred.

Responsibilities

• Identifies program participants through outreach and helps families access health and human services (prenatal care, WIC, Medicaid, etc.).
• Builds relationships with community members and leaders to gain program support and referrals.
• Carries a caseload of families with regular client contact to establish trust.
• Conducts interviews to identify needs and referrals for housing, food assistance, transportation, day care, etc.
• Creates case plans with goals and activities based on family needs.
• Assists parents in scheduling appointments and acts as liaison with service agencies.
