Job summary

Henry Ford Health is seeking a Community Health Worker for the WIN Network to co-facilitate Group Prenatal Care sessions, coordinate logistics, and engage participants while identifying patients with risk factors and facilitating referrals.

The CHW provides education on prenatal care, addresses social determinants of health, links patients to community resources, and acts as liaison between patients and the clinical team to promote adherence.

Qualifications

• High school diploma or GED required.
• Previous CHW experience preferred.
• Experience delivering health education to underserved populations preferred.
• Experience conducting home and/or clinic visits preferred.

Responsibilities

• Co-facilitates Group Prenatal Care (GPC) sessions, coordinating activities, logistics, and guest speakers.
• Identifies patients with clinical risk factors and refers to appropriate programs.
• Delivers prenatal care education and health coaching on nutrition and chronic disease management.
• Conducts SDOH screenings to identify barriers and supports integration into care plans.
• Coordinates connections to community resources and ensures follow-up for referrals.
• Acts as liaison between patients and the clinical care team to promote engagement and adherence.
