Job summary

Atlantic Health System in Morristown, NJ seeks a Care Coordination Specialist for the Atrial Fibrillation Center to coordinate scheduling, pre-operative testing, and patient education to ensure smooth, safe procedures and timely recovery.

You will work closely with EP physicians and nursing staff to align plans, communicate with patients and families, and monitor post-procedure follow-up. A compassionate, organized approach is essential to support patients through the AFib care pathway.

Responsibilities

• Procedure scheduling: Manage scheduling logistics for AFib Center procedures (catheter ablation, LAA closure) and coordinate with departments; inform patients and families.
• Patient assessment and preparation: Coordinate pre-operative testing and review results to ensure readiness; evaluate history and tests.
• Patient education and support: Educate patients and families on the procedure, pre-procedure prep, and post-procedure care; provide compassionate support.
• Follow-up care coordination: Oversee post-procedure follow-up, schedule appointments, monitor recovery, ensure guideline adherence, escalate issues.
• Interdisciplinary collaboration: Work with EP physicians and other professionals to develop care plans.
• Performs other related duties as assigned.
