• Follow the nursing process: assess, plan, implement, evaluate.
• Assessment – systematic collection and analysis of psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors.
• Plan – set measurable short- and long-term goals based on assessment, in collaboration with patient and multidisciplinary team.
• Implement – execute and modify care plans using assessment data, clinical reasoning, evidence-based practice; coordinate care, educate, discharge planning; delegate per professional and legal standards.
• Evaluate – systematically assess progress toward goals and revise plan as needed with patient and team.
• Document all phases of the nursing process in the patient record.
