The Role:

Under general supervision, the Clinical Coordinator Assistant-Transplant supports RN Coordinators in pre and post-care phases, working with labs, imaging, registration, physician offices, and financial services. Responsibilities include facilitating patient flow, coordinating referrals, evaluations for therapy, organ recipients, donors, VAD patients, and ongoing patient care and education. The Assistant initiates actions in electronic records to ensure process adherence and volume metrics, providing excellent patient service, respecting confidentiality, and maintaining professional relationships.

Team Contributions:

• Communicates effectively with patients, families, and visitors with courtesy and tact
• Acts as liaison between the program and support/clinical staff, triaging calls, providing updates, and managing concerns
• Serves as primary contact between the program and referring community, managing calls, providing information, and handling correspondence
• Handles referral processing, including reviewing inputs, verifying demographics and insurance, assessing appropriateness, gathering medical records, and ensuring timely processing within 14 days

Qualifications:

• At least 1 year of experience in a complex healthcare environment such as catheter labs, endoscopy, infusion centers, or inpatient care within the last three years
