Job summary

Kickapoo Traditional Tribe of Texas invites applications for the PRC Facilitator position within the Health & Human Services Administration. This in-house only role supports the PRC program and provides transit services to KCHC patients.

The PRC Facilitator will work independently, manage multiple projects for the PRC Manager, ensure safe vehicle operations, assist elderly clients with ambulation, and protect patient confidentiality.

  • Experience with health care settings is a plus.

Position: PRC Facilitator

Supervisor: PRC Manager

Division: Health & Human Services Administration

Responsibilities:

This position will provide transit services with Kickapoo Community Health Center (KCHC) Purchased/Referred Care (PRC) Program patients in effort to provide comprehensive transit services. The goal of this effort is to provide an organized and wide-ranging approach for transit services to meet the needs of all KTTT community. The PRC Facilitator must work independently and manage multiple projects assigned by the PRC Manager. This is a confidential position, and discretion must be exercised in protecting and releasing information.

Essential Duties and responsibilities include the following:

  • Distributes referral and assistance services according to patient need or assignment.
  • Ensure transit vehicles are maintained and reliable.
  • Facilitate communication between patients and providers to enhance accessibility of health care facilitates.
  • Follow established Tribal policies and those outlined in the Kickapoo Traditional Tribe of Texas Employee Handbook.
  • Interpret language when the language used by health care providers differs.
  • Maintain a referral system: keep a centralized system for medical referrals.
  • Operations of the vehicle in a safe and courteous manner with regard to traffic safety regulations and comfort of passengers. Will follow stated speed limit.
  • Participates in required training programs and attends all staff meetings as required.
  • Physically able and willing to assist elderly clients with ambulation, including loading and uploading of wheel/chair bound patients.
  • Pick-up and delivery of medications from non-KCHC pharmacies.
  • Provide local and out of town transit services where necessary for health care.
  • Provides client/patient reminder slips prior to appointment of upcoming appointments to minimize the no show rate.
  • Provides superior quality customer service to ensure that each person who requests service is treated with dignity, respect, and patience.
  • Speak up for patients who are being referred for outside services.
  • Treat patients with dignity, respect and patience.
  • Will complete and maintain required records and receipts for reimbursement
  • Other duties may be assigned at the discretion of the PRC Manager or designee to fulfill the Department’s needs, objectives and/or goals.

Knowledge, skills and abilities for the position:

  • Ability to answer intelligently, tactfully and accurately the question(s) raised by patients.
  • Ability to exercise independent and sound judgment.
  • Ability to manage and maintain security of confidential data and information.
  • Ability to perform simple arithmetic for time keeping and counting calculations.
  • Ability to read and write English and Spanish.

  • Computer experience in Microsoft Office applications required.

  • Knowledge of health billing procedures and terminology.

  • Work effectively as an individual and team member.

Supervision exercised:

This position does not carry out supervisory responsibilities in accordance with the organization’s policies and applicable laws.

Education/experience requirements:

Minimum Qualifications: High school diploma or graduation equivalent diploma (GED) required.

Must obtain certifications in Cardiopulmonary Resuscitation (CPR), Automated External Defibrillator (AED) and First Aid.

Preferred Qualifications: An associate’s degree or related healthcare work related experience is preferred.
