Practice Transformation Advisor at CVS Health
Join CVS Health as a Practice Transformation Advisor and contribute to transforming healthcare delivery. This role involves supporting provider practices in improving quality and efficiency, analyzing performance metrics, and fostering collaboration across teams.
Responsibilities
- Provider Engagement: Establish and maintain relationships with large provider groups (membership of 1,000+).
- Performance Analysis: Analyze provider performance metrics, clinical quality indicators, patient and provider satisfaction, and operational efficiency to identify improvement opportunities.
- Provider Education and Practice Transformation: Collaborate with teams to assess practices, design training programs, and facilitate sessions on clinic operations, quality standards, and patient-centered care.
- Performance Improvement Initiatives: Lead efforts to develop and monitor performance improvement plans.
- Quality Assurance: Conduct audits and reviews, providing feedback for improvement.
- Stakeholder Collaboration: Work with internal teams to align performance objectives and support provider success.
- Data Management and Reporting: Use analytics tools to report on performance data, preparing reports and dashboards for leadership.
- Industry Knowledge and Research: Stay updated on healthcare trends, regulations, and best practices to support quality and health equity initiatives.
Minimum Requirements
- 5-7 years of experience in healthcare quality improvement or provider relations.
- Knowledge of healthcare regulations, quality standards, and performance metrics.
- Strong analytical, communication, and interpersonal skills.
- Experience in training and data analysis tools.
- Familiarity with electronic health records and healthcare IT.
- Ability to manage multiple projects independently.
Preferred Qualifications
- Licensed Allied Health Professional or Registered Nurse.
- Coding or Sales background.
- Certified Six Sigma Green Belt or CPHQ.
- NCQA PCMH Content Expert certification.
Additional Details
- Position is remote; secure home network required.
- Bachelor's degree or equivalent work experience in healthcare or related field.
- Full-time, 40 hours/week.
- Salary range: $66,330 - $145,860, with potential bonuses.
