Job summary

Dorset HealthCare University NHS Foundation Trust is seeking a Practice Nurse to assess, plan, implement and evaluate nursing care in line with the NMC code of conduct. The role involves long-term condition management, health promotion, MDT collaboration and assistance with minor procedures in the community setting.

Responsibilities include promoting patient self-care, maintaining accurate Read Codes, and contributing to NHS health checks and immunisation programmes as part of routine practice.

Qualifications

• NMC code of conduct awareness
• Ability to assess, plan, implement and evaluate nursing care

Responsibilities

• Long Term Conditions: participate in annual care of patients with long-term conditions and promote self-care.
• Support QOF prompts and documentation with Read Codes.
• Promote integrated MDT working and referrals to wider health and social care services.
• Use SystmOne for managing patient records and data extraction for monitoring and audit.
• Deliver health promotion and screening, including NHS health checks and BP checks.
• Assist in minor surgery sessions and other procedures as required.
• Carry out cervical cytology sampling under supervision and perform venepuncture and ECGs.
