Jordan Health is seeking a Practice Manager to lead the Holland Street Adult Medicine and Urgent Care Center. The PM will supervise clinical support staff and collaborate with providers to ensure high-quality, patient-centered care in a Federally Qualified Health Center setting.

The role emphasizes operational excellence, regulatory compliance, and population health advancement within a multi-site network dedicated to serving Rochester, NY communities.

Qualifications

• Minimum two years of management experience.
• Clinical experience with Family Medicine/Behavioral Health preferred.
• Bachelor's Degree in nursing or an allied health field with supervisory experience preferred.
• Excellent computer skills required, preferably with an electronic health record.

Description

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.

Jordan Health is now seeking a Practice Manager (PM) who, under the guidance of the Director of Nursing, is primarily responsible for providing leadership, oversight, and management of Anthony L. Jordan Health's Holland Street Adult Medicine and Urgent Care Center. This role is accountable for the operational, clinical, and performance management of services delivered within Federally Qualified Health Center (FQHC) environment. The PM supervises clinical support staff and works collaboratively with providers and interdisciplinary team members to ensure the delivery of efficient, high-quality, patient-centered care aligned with the mission of Anthony L. Jordan Health Corporation.

Education And Experience Required:

• Minimum two years of management experience.
• Clinical experience with Family Medicine/Behavioral Health preferred.
• Preferred: Bachelor's Degree in nursing or equivalent allied health field with experience as a manager/supervisor.
• Excellent computer skills a must, preferably with an electronic health record.

Licenses And Certifications Required:

• Valid NYS RN License with no negative actions.
• Current provider BLS/CPR through the American Heart Association.
• Infection Control Certificate.
• NYS RN Registration Certificate
• Valid NYS driver's license and reliable transportation
