Job summary

TASC Treatment Alternatives for Stronger Communities is seeking a full-time Case Manager. You will support individuals across multiple counties and travel as required to Will, Lake, McHenry, DeKalb, DuPage, Kankakee, Kendall and Kane counties.

Starting pay ranges from $44,676 to $47,000 depending on experience and education. The role involves intake processing, crisis intervention, referrals, and collaboration with professionals to monitor progress.

Qualifications

• High school diploma or GED required.
• Valid driver's license required.
• CADC certification within two years may be required based on contractual obligations.
• Pass an IDOC background check.
• Knowledge of DSM V and ASAM criteria.

Responsibilities

• Assess, evaluate, facilitate and monitor individuals progress toward goals.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention for identified needs.
• Interview individuals, review records, and confer with professionals on suitability.
• Collaborate with internal and external partners and participate in supervision.
• Identify and refer individuals with behavioral health needs.
• Complete and maintain all required documentation and reports.
• Monitor enrollment and progress via required systems.

Position Summary

We are currently looking for full time Case Manager Candidate must be willing to travel to the following Counties Will Lake McHenry DeKalb DuPage Kankakee Kendall and Kane Starting at 44676 47000 contingent upon experience education etc

Essential Duties and Responsibilities

• Assess evaluate facilitate and monitor individuals progress in relationship to established goals and objectives
• Complete intake packets for program enrollment and participation
• Provides referrals and crisis intervention for the individuals identified needs
• Interview individuals review records and confer with other professionals to review individual progress and to determine their suitability for program participation
• Knowledge of DSM V and ASAM criteria
• Collaborate with internal and external partners participate in staff meetings and monthly supervisions and participate in integrated staffing with individuals receiving TASC services
• Identify and refer individuals with behavioral health needs
• Complete and submit internal and external program reports
• Complete and maintain all required individual and program related documentation which includes documenting the results of screenings enrollments service plans reports progress notes discharge summaries and other individual related activities including accurate reporting of services
• Monitor all individuals determined by individual progress towards service goals
• Monitor TASC database system and contractualexternal database systems to ensure Individual enrollment

Ideal Candidate

• High school diploma or GED equivalent
• Valid drivers license
• CADC certification within two years of employment with TASC may be required based on contractual obligations
• Must be able to pass an IDOC Background check
• Demonstrated proficiency in the Microsoft Office suite
• Demonstrated experience in group facilitation
• Knowledge of the current ASAM placement criteria DSM V as it relates to substance related disorders and Code of Ethics for substance abuse professionals or the ability to acquire the knowledge
• Comfort working inside a correctional setting and with persons convicted of serious violent offenses
• Flexibility in thinking and approaches to leading group discussions
• Good verbal written and interpersonal communication skills
• Good time management skills with a proven ability to meet deadlines

The agency currently offers hybrid work schedules that combine in office and remote work Employees are required to report to the work site three days per week with the option to work from home to two days remotely after 60 days of employment contingent upon job duties
