Job summary

National Health Foundation is seeking a Social Service Coordinator in Glendale to provide post-hospital care for guests experiencing homelessness, coordinating services and ensuring program goals are met. The role requires strong communication, organizational skills, and the ability to manage a caseload of 25+ guests.

The position is non-exempt, full-time, with an hourly rate of $26.44 and a Tuesday–Saturday shift.

Qualifications

Bachelor’s degree required; Master’s in social work preferred.
Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or similar field.
Bi-lingual Spanish preferred.
Excellent communication skills both oral and written.
Time management and strong organizational skills.
Fingerprint live scan / background may be required.

Responsibilities

Provide post-hospital care services to guests transitioning out of acute care hospital.
Complete needs assessments and care plans; coordinate referrals for mental health, housing, and other services.
Facilitate discharge process and locate housing; communicate with community partners.
Maintain CRM database and document in HMIS and CES as required.
Engage guests daily, build rapport, and manage a caseload of 25+ guests.
