Job summary

CarePartners Health Services, a member of Mission Health within the HCA Healthcare family, seeks a Post-Acute Transitions Specialist in western North Carolina. The role collaborates with hospitals, SNFs, and physician practices to coordinate transitions, educate teams, and promote seamless admission processes across post-acute programs.

The specialist will work closely with post-acute providers and care managers to maximize positive patient outcomes, support optimal placement, and support goals

Qualifications

  • Bachelor of Science in Nursing or related field.
  • RN license in North Carolina with NC BON (eNLC may apply).
  • 2 years experience in related field; 8 years preferred.

Responsibilities

  • Coordinate between internal and external referral sources and CarePartners Post-Acute Service Lines.
  • Establish relationships, provide education, and promote collaboration with hospitals, SNFs, SNF discharge planners, and care managers.
  • Ensure appropriate patient placement through screenings and assessments with interdisciplinary teams and care coordinators.
  • Collaborate with health system and community colleagues to ensure quality care and optimize transitions to reduce readmissions.

Job Summary And Qualifications

This position acts as a coordinator between internal and external referral sources and CarePartners Post-Acute Service Lines. The Post-Acute Transitions Specialist is responsible for establishing relationships, providing education, and promoting collaboration with hospitals, skilled nursing facilities, assisted living facilities, ambulatory and inpatient care managers, physicians, payors, MHP CIN, and other community referral source. The Post-Acute Transitions Specialist will ensure appropriate patient placement through screenings and assessments in collaboration with SNF Discharge Planners & Inter- Disciplinary Teams, Ambulatory Care Managers, MHP Care Managers, BPCI Transitions Care Coordinator, practice referral coordinators, physicians, and CarePartners PostAcute service line team members.

The Post-Acute Transitions Specialist works closely with Post-Acute providers, SNF’s, and physician practices in the clinically integrated network (CIN), hospital care managers, Mission Palliative Care team, etc. to ensure a responsive and smooth flowing referral and admission process for CarePartners Post-Acute programs. They will collaborate with health system and community colleagues to insure quality driven, best practice across the continuum by utilizing essential activities of care management in direct patient contact. They will maximize positive financial outcomes for designated patient populations, intervening at key points in the disease process by facilitating appropriate Post-Acute placement, participating in Goals of Care/Advanced Care Planning dialogue and effective transitions to prevent readmissions, and lower total cost of care thru the appropriate utilization of PostAcute services.

What qualifications you will need:

  • Bachelor of Science in Nursing; Degree in Physical, Occupational, or Speech Therapy; Social Work; Psychology; Business; Health Sciences or related field
  • If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse Licensure Compact) license may apply, licensee should confirm with NCBON. If applicable, acquires and maintains National Nursing Certification in Case Management (ACM or CCM, or ANA approved certification). If applicable, license in therapy discipline.
  • 2 years experience in related field, 8 years experience preferred.
