Job summary

Desert Springs Post Acute is seeking a Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team in Riverside County, CA. CFY candidates are encouraged to apply and will receive mentorship in a supportive skilled-nursing setting.

The role requires evaluating and treating residents with speech, language, cognitive, and swallowing disorders, developing personalized plans, and collaborating with physicians and staff to achieve optimal outcomes.

Qualifications

• Master's Degree in Speech-Language Pathology from an accredited program.
• Current California SLP license, or eligibility for licensure.
• CFY candidates welcome and encouraged to apply; CCC-SLP preferred for licensed clinicians.
• Current CPR certification preferred.
• Skilled nursing facility experience preferred but not required.

Responsibilities

• Evaluate residents for speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans based on clinical findings and physician orders.
• Provide skilled therapy services to improve communication, cognition, and swallowing function.
• Assess residents for dysphagia and recommend appropriate interventions and diet modifications.
• Collaborate with nursing, dietary, rehabilitation staff, physicians, and families to ensure quality care.
• Maintain timely and accurate documentation in accordance with facility, state, and federal guidelines.
• Participate in interdisciplinary care planning and discharge planning processes.
• Educate residents, families, and caregivers on treatment strategies and recommendations.
• Monitor resident progress and adjust treatment plans as needed.
