Qualifications

• Current Massachusetts physician license and DEA registration.
• Experience in post-acute, acute care, ED, or clinic settings preferred; new grads welcome.
• Excellent communication and interpersonal skills; collaborative, team-oriented approach.

Responsibilities

• Perform comprehensive patient assessments and medical histories.
• Develop and implement individualized care plans for post-acute patients.
• Monitor progress and coordinate care with physicians, nursing staff, and other healthcare professionals.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate medical records in compliance with regulations.
