You will deliver high-quality care to a diverse patient population, collaborate with a multidisciplinary team, and participate in onboarding and mentorship programs to support your transition into post-acute medicine and ongoing

Qualifications

• Current physician license (State of Indiana) and DEA.

Responsibilities

• Perform comprehensive patient assessments, including medical histories and physical exams.

• Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
• Educate patients and families on health management and preventive strategies.
