Luminis Health – LH Ambulatory – Care Mgmt in Annapolis, MD seeks a Performance Specialist for the Post-Acute Network. This role engages facilities, reviews throughput and readmission metrics, and coordinates resolution with SNFs, IRFs, LTACHs, and home-based partners.

The specialist supports the post-acute strategy, identifies barriers, and collaborates with leadership to drive improved transitions and performance across the network.

Qualifications

• Bachelor’s degree in Healthcare Administration, Public Health, Social Work, or related field.
• 3–5 years experience in post-acute care, care management, hospital operations, or performance improvement.
• Experience engaging executive and external partner stakeholders.

Responsabilités

• Conduct Facility-Level Performance Reviews with post-acute partners.
• Monitor partner performance trends in throughput and readmissions.
• Identify barriers and coordinate resolution plans with leadership.
• Promote throughput reliability through improved documentation and discharge progression.
• Reinforce network performance standards via regular engagement.
• Discuss clinical alignment and high-impact DRG trends with leadership.
• Maintain structured performance documentation and logs.
• Escalate systemic trends to inform enterprise decisions.
• Assist onboarding of new partner facilities into governance model.
